What is the programme?
The HSE Chronic Disease Management (CDM) Programme helps people with certain long-term conditions stay as well as possible. Instead of only seeing your GP when something goes wrong, you have regular planned reviews to keep your condition under control and catch any problems early.
Who can join?
Adults aged 18 and over who hold a medical card or GP visit card and have been diagnosed with one or more of the following:
-
Type 2 diabetes
-
Asthma
-
COPD (chronic obstructive pulmonary disease)
-
Heart failure
-
Atrial fibrillation
-
Ischaemic heart disease (such as angina or a previous heart attack)
-
Stroke or TIA (mini-stroke)
-
High blood pressure (hypertension)
What happens at a review?
You will usually be invited for two planned reviews each year. Your visit may include:
-
Blood pressure, weight and other measurements
-
Blood tests, booked ahead so results are ready for your review
-
A review of your medicines
-
Advice on diet, exercise, smoking and alcohol
-
A personal care plan agreed with you
-
Referral to other services where needed
Prevention Programme
If you are at high risk of developing a chronic condition, for example if you have pre-diabetes or a high risk of heart disease, you may be eligible for the CDM Prevention Programme. This includes a yearly review to help lower your risk.
Opportunistic Case Finding
Eligible patients aged 45 and over who have not been diagnosed with a chronic condition may be offered a check for conditions such as high blood pressure, diabetes and heart disease when they visit the practice.
Is there a cost?
No. Programme reviews are free for eligible medical card and GP visit card holders.
